If you missed a meeting, you must print this and take it with you to the make up meeting you schedule with your buddy. Attach it to the make up meeting form and turn it in.
Make Up Mentor Buddy Meeting Form
If a mentor or buddy misses a meeting they must phone the other party and plan a common meeting time and location within one week following the missed Mentor Buddy Meeting. The Buddy parent must be in attendance. The mentor should print the Mentor-Buddy Meeting Check list for the specific meeting date that was missed. Mentors should check for this online under the Mentor Heading. Attach the completed check list to the Make up meeting form and turn in to Mrs. Carothers mailbox or the SA mailbox in room #500.
